It’s a program that pairs you with a clinical health coach who is an experienced NHS registered nurse who gives you information, advice and encouragement over the phone to help you set and reach health goals.

Health coaching happens on the phone in two stages:

  • Your first coaching session: Your initial phone call will take 45 - 60 minutes. You and your coach will undertake and review your ‘health risk appraisal’. Together, you’ll develop personal goals and an action plan.
  • Your regular sessions: You’ll speak with your coach by phone for 15 - 30 minutes, usually weekly, to review your progress toward your goals, explore and resolve challenges and set your goals for the next week
  • No call limits: you are also free to call your coach at any time, as often as you like

Clinical health coaching works well over the telephone for a variety of reasons and is an effective and practical means of accessing support. Delivered at a time and place convenient to you, it can provide an increased sense of control by eliminating some of the barriers and intimidations that are often present during physical interactions with healthcare staff, resulting in higher levels of engagement and better outcomes from the coaching process.

It’s usually a 12 month programme followed by a 6 month step-down, but it could be longer or shorter based on your personal needs. At the end of your 12 month subscription you will discuss with your coach if you would benefit from continuing on the programme, would like to move onto the step down service or are ready to leave the programme entirely. You can also move on to a maintenance programme, which offers regular information and direct access to your previous coach. If for any reason you want to leave the programme before 12 months, you can do so at any time.

We don’t provide member-specific information to your doctor or anyone else without your permission. If you agree to us discussing with your doctor we will notify them you have enrolled and will send a discharge report at the end of coaching.

It is worth remembering that by its nature coaching brings greater improvement with time. If however you decide coaching isn’t for you, you can leave the program at any time and simply cancel your subscription.

Your health is your most important asset. Working with an experienced, certified clinical health coach gives you resources and support to help you be your healthiest. You may also want a health coach if:

  • You’re running low on motivation. A clinical health coach can help you build confidence, provide support and offer inspiration.
  • You need someone on your side. Your health coach is like a really knowledgeable friend who’s there to listen, ask good questions and keep you on track with your goals.
  • You need help recognising successes. A health coach can help you identify wins and celebrate your progress.
  • You need extra support. Health coaching can enhance and support other wellness programs you participate in such as Weight Watchers® or fitness classes

Our clinical health coaches are trained nurses with a minimum of 5 years NHS experience, who are experts in your condition. They have specialist training and skills in health coaching and shared decision making.

The aim of health coaching is to empower you to take control of your health and reduce the impact of the long-term condition on your daily life. By increasing your understanding, knowledge and confidence we will help you enjoy the success of managing these situations independently.

Our coaching is delivered by telephone only. This is the most practical, efficient and positive way of delivering clinical health coaching and is the best way to help you increase your independence and self-confidence.
